Safety Management Systems (SMS) Occurrence Investigation Fundamentals

Course Overview

This training program aims to equip relevant aviation safety professionals with the fundamental knowledge and skills to investigate aircraft occurrences within an operator’s Safety Management System (SMS).

As part of Safety Risk Management training the aim of the cause is to enhance the identification of causes and implementation of robust preventative measures of occurrences. Our experienced instructors will guide participants through a blend of interactive lectures, classroom exercises, and real-world case studies.

  • Handling Occurrences within the SMS
  • Objective and process of investigation
  • Investigation Authority, Responsibility and Roles
  • Reporting of occurrences
  • Participation in the investigation
  • Initial actions and site safety
  • Handling of recorders
  • Aircraft Systems investigation overview
  • Human factors investigation overview

Target Group


    • Safety managers and officers
    • In-house occurrence investigators
    • Air Operator emergency response personnel (go team, Crisis Management Centre (CMC) team, Local Incident Command Centre (LICC) team
    • Auditors who audit organisations’ SMS

Prerequisites


    • Participants should have preferably completed the Aerosafe Safety Management Systems for Managers course (5 days)
    • Recommended level: this is an intermediate level SMS course
    • Computer skills: working knowledge of office productivity software

Required Materials


    • Participants must bring to class a laptop which has functional office productivity software (Microsoft 365/Google Workspace etc.) and internet connection capability

Qualification Standards


    • Successful completion of case study and exercises
    • Exam – 70% pass mark
